Medical Coverage

Medical insurance coverage is used to pay for eye exams when:

• There are specific concerns that are medical in nature.

• There is a previously diagnosed medical condition of the eye.

• Health conditions are present that may damage the eye.

Some examples are:

• Lazy Eye

• Crossed Eyes

• Glaucoma

• Diabetes

• High Blood Pressure

• Juvenile Arthritis

• Eye problems related to  premature birth

• Autoimmune disease

• Headaches

Usually, you cannot use your routine vision coverage if you are being treated for a medical condition. You will need to bring some documentation from your plan that allows us to bill your plan for your care.

Check with your plan to see if:

• Dr. Waeltermann or Dr. Bomse are in your plan or if you can go to any doctor       you choose.

• You need a referral. If you need a referral, please bring the referral to your appointment.

• You have a Specialist copay and the amount of the copay
